Nephrotic syndrome

NEPHROTIC SYNDROME

Nephrotic Syndrome called kidney disorder which led your body to excrete too much protein with your urine. Symptoms are like foamy urine, swelling around the eyes and in the feet and ankles, weight gain due to excess fluid retention.

Children are mostly affected and the disease is rare in adults. If not treated properly the kidney function deteriorates gradually resulting in kidney failure. So proper treatment is the mainstay to prevent further damage to the kidneys. As it is a rare and unique disease of only childhood, hence without opting for general child specialist or adult nephrologist, you should consult experienced pediatric nephrologist for better management.

No limitations need to be placed on kidney patients sexually. If the sexual activity does not place pressure or tension on the access site, it will not cause damage. Fear can cause people to avoid sexual activity needlessly.

Mismanaged Nephrotic Syndrome

Mismanaged Nephrotic Syndrome

The mainstay of treatment of nephrotic syndrome is corticosteroid. However, as much as 80% of these children experience at least one recurrence and around 40% develop a more complicated course with frequent recurrences. Repeated and prolonged courses of steroids in these children often result in long-term complications spanning from diabetes, hypertension to cataract and facial defformity. Hence, steroid sparing agents such as cyclophosphamide, tacrolimus, levamisole, and mycophenolate mofetil (MMF), Rituximab are being increasingly used. As each of these drugs has specific indication and recommendation to use, you should consult experienced pediatric nephrologist for better management.

Urinary tract infections in children

URINARY TRACT INFECTION (UTI)

Urinary tract infection (UTI) is one of the most common childhood infections. Permanent renal scarring may occur in affected children, especially with recurrent UTIs, leading to long-term complications such as hypertension and chronic renal failure. Recurrent UTIs sometimes happen along with other conditions, such as:

  • a) vesicoureteral reflux (VUR), which is found in 30%–50% of kids diagnosed with a UTI. In this congenital (present at birth) condition, pee flows backward from the bladder to the ureters. Ureters are thin, tube-like structures that carry pee from the kidney to the bladder. Sometimes the pee backs up to the kidneys. If it’s infected with bacteria, it can lead to pyelonephritis.
  • b) Hydronephrosis, which is an enlargement of one or both kidneys due to backup or blockage of urine flow. It’s usually caused by severe VUR or a blocked ureter. You should consult experienced pediatric nephrologist for best management.

Hydronephrosis

HYDRONEPHROSIS

Hydronephrosis means that the kidneys are abnormally dilated or overfilled with urine. There are several causes, many of which do not cause any harm to the kidneys, but in some cases, hydronephrosis can be associated with poorly functioning kidneys. Hydronephrosis can affect one (unilateral) or both (bilateral) kidneys.

Hydronephrosis can be caused by a blockage at some part of the urinary tract or by reflux of urine (abnormal backflow of urine from the bladder). It can also be due to the abnormal development or formation of the kidney in the absence of obstruction or reflux. There are few genetic causes of hydronephrosis and it can occur with other conditions in the baby. There are three major categories of hydronephrosis:

Vesicoureteral reflux is when urine does not properly flow from the kidneys to the bladder, but instead abnormally backs up in the ureter (the tube connecting the kidneys to the bladder).
Blockage/obstruction occurring in four possible places:
Where the kidney meets the ureter (ureteropelvic junction (UPJ)
Where the ureter meets the bladder (ureterovesical junction (UVJ)
Within the urethra (the tube that carries urine from the bladder out of the body, occurring only in males) (posterior urethral valves (PUV)
Incorrect attachment of the ureter to the bladder (ectopic ureter or ureterocele)
Idiopathic hydronephrosis that has no obvious cause and usually resolves on its own before or after birth.
You should consult experienced pediatric nephrologist for best management.

Acute kidney injury

Acute renal failure / Glomerulonephritis

Acute kidney failure occurs when your kidneys suddenly become unable to filter waste products from your blood. When your kidneys lose their filtering ability, dangerous levels of wastes may accumulate, and your blood’s chemical makeup may get out of balance. Acute kidney failure — also called acute renal failure or acute kidney injury — develops rapidly, usually in less than a few days. Acute kidney failure is most common in people who are already hospitalized, particularly in critically ill people who need intensive care. It may also be the outcome of acute Glomerulonepritis.

Acute kidney failure can be fatal and requires intensive treatment. However, acute kidney failure may be reversible if treated properly and promptly.

Chronic kidney disease

Chronic kidney disease

Chronic kidney disease (CKD) is a major health problem worldwide. Although relatively uncommon in children, it can be a devastating illness with many long-term consequences. Chronic renal failure progresses slowly over at least three months and can lead to permanent renal failure. So the mainstay treatment is to slow the progression of the disease. You should consult experienced pediatric nephrologist for best management.

Hemolytic uremic syndrome (HUS)

Hemolytic uremic syndrome (HUS)

Hemolytic-uremic syndrome (HUS) is a clinical syndrome characterized by progressive renal failure that is associated with anemia and thrombocytopenia. HUS is one of the most common causes of acute kidney injury in children. It may be typical (with diarrhea) or atypical (without diarrhea). Atypical one are more common and devastating. The disease needs urgent aggressive management to save both kidneys from permanent damage. You should consult experienced pediatric nephrologist for best management.

Hematuria

Hematuria

Hematuria is the presence of red blood cells in the urine. Visible hematuria, also known as gross hematuria, is easily identified, as it causes red or brown discoloration of the urine. It may be due to Kidney stone, UTI, renal injury etc. The disease needs urgent aggressive management to save both kidneys from permanent damage. You should consult experienced pediatric nephrologist for best management.

KIDNEY TRANSPLANT & KIDNEY FAILURE

It is always better to delay kidney transplant as much as possible as transplanted kidneys generally do not work for more than 20 years even if all goes well. So, if transplant done in a child of 5 years, it is expected that at the age of 25 years he/she again needs re-transplantation. There is also issues of donor selection and legal procedures. Aside this, there are chances of kidney rejection, infections etc. Moreover, cost of medicines after transplant is usually much more during dialysis. However, transplant is the last resort when other options are already exhausted.
